PROJECT VISITOR SAFETY ORIENTATION FORM

This form is to be completed before a vistor enters the site.

Please complete all sections that are applicable to your company vist and worksite activities.

This form must be signed and dated by the individual faclititating the orientation and the visitor who has revieved the site oritentation.

Once signed, the original form will be retained as documentation.

I agree to all of the site rules and code of conduct while on-site.

Visitors must sign in and sign out at reception.

Visitors must attend the basic safety induction provided.

Visitors must be accompanied by a member of our staff at all times.

Visitors must follow the directions provided by our staff.

Visitors must not use scaffolding, ladders, or equipment.

Visitors must observe all site signs and directions for your own safety.

Visitors must not cross any safety barrier.

All visitors must wear PPE (Personal Protective Equipment) provided - Vest, Helmet, safety boots (if required) or appropriate flat soled shoes.

All visitors must wear appropriate clothing ( no high heels, reveiling or loose flowing clothing).

You must be accompanied by a staff member at all times.

Do not use your mobile phone (only in designated areas).

Smoking is NOT allowed on the site.

Report and accidents or near misses immediately to a staff member.

Site Plan review - You have completed a review of the site plan including exit routes.

The Issued ID must be worn at ALL times and returned on completion of the visit.
